Statins: what they do, what they don’t and the evidence on alternatives

Two people. Same cholesterol number. Very different risk.

As a Registered Nutritional Therapist specialising in cardio-metabolic health, I see this most weeks. Someone hands me their cholesterol result and makes me aware of their statin prescription, but confirms they have had very little explanation of either.

Statins are one of the most commonly prescribed medicines for cholesterol, and they are often misunderstood. Most people think of them as “cholesterol-lowering tablets” but their real value should be with reducing the risk of heart attack and stroke.

So how do statins actually work? Who benefits most? What side effects are worth knowing about? And what does the evidence say about lowering cholesterol without them?

What statins actually do

Statins work by blocking an enzyme in the liver called HMG-CoA reductase. This reduces the liver’s own cholesterol production and lowers the amount of cholesterol inside liver cells.

In response, the liver makes more LDL receptors on its surface. These receptors help remove more LDL cholesterol from the bloodstream, which lowers the amount of classically-considered “bad” cholesterol circulating in the body.

However, this process is why statins are best understood as cardiovascular risk-reducing medicines, not just cholesterol-lowering drugs. They may also help by improving blood vessel function, reducing inflammation and stabilising plaques in the arteries.

Who benefits and what NNT means?

Statins are used for both primary and secondary prevention.

Primary prevention is for people who do not already have cardiovascular disease but are at risk of a first heart attack or stroke. This includes people with a QRISK of 10% or more, people with type 1 or type 2 diabetes, chronic kidney disease, familial hypercholesterolaemia and some older adults.

Secondary prevention is for people who already have established cardiovascular disease – most commonly, they have had a heart attack. Here, statins reduce the risk of another event and are usually recommended regardless of cholesterol level.

For people with a QRISK below 10%, NICE says the decision should be individual – a discussion about likely benefits, possible downsides and personal preferences.

The Number Needed to Treat (NNT) describes how much benefit a treatment is likely to give. At low risk the benefit is smaller, so the NNT is less favourable. That does not mean statins never help; it means the case is much stronger when risk is already higher.

This is exactly the point where most people feel stuck. Your QRISK, your family history, your blood sugar and your full lipid panel all change the maths – and a short GP appointment rarely has time to walk through them. Side effects and safety

The most common side effect associated with statins is muscle-related symptoms, often called statin myopathy: muscle pain, weakness, cramping, fatigue and sometimes tendon problems.

  • Myalgia means muscle pain, with or without raised creatine kinase.
  • Myositis means muscle pain or weakness with raised creatine kinase.
  • Rhabdomyolysis is a very rare but serious form of muscle damage.

The exact reason statins can cause muscle symptoms is not fully understood. One possible link is reduced coenzyme Q10 production, since statins affect the same pathway involved in making CoQ10, and there is evidence that CoQ10 alongside statin therapy can ease symptoms of myopathy. There is also a small possible risk of type 2 diabetes, mainly relevant in people already at higher risk.

 

Lowering cholesterol without statins

Yes – in many cases, lifestyle changes make a meaningful difference. Five interventions have the strongest evidence.

  1. Diet: One of the most effective strategies is increasing soluble fibre – oats, beans, lentils, flaxseeds and psyllium husk. Soluble fibre lowers LDL by reducing its absorption in the gut and interrupting bile acid recycling. Around 5 to 10g of soluble fibre a day can lower LDL by roughly 5 to 10%. Including more unsaturated fats (mono- and polyunsaturated) helps clear LDL from the blood, while limiting ultra-processed foods and added sugar matters too: refined carbohydrates and sugars are linked to higher triglycerides, more insulin resistance and a greater share of small, dense LDL particles – the kind most strongly tied to cardiovascular risk.
  2. Exercise: Regular activity raises HDL, lowers triglycerides and improves the liver’s ability to clear LDL. Aerobic exercise – walking, cycling, swimming – is particularly effective; around 150 minutes of moderate activity a week is a sensible benchmark, with two to three strength sessions added for insulin sensitivity.
  3. A healthy weight: Reducing visceral fat improves insulin sensitivity, lowers triglycerides and lifts HDL. Even 5 to 10% body-weight loss has been linked to measurable improvements in cardiovascular risk markers within a year.
  4. Stopping smoking: Smoking oxidises LDL, lowers HDL and damages the blood vessel lining. HDL can begin to recover within weeks of quitting
  5. Alcohol: Alcohol is converted to triglycerides in the liver and, in excess, raises LDL and cardiovascular risk over time. Current UK guidance is no more than 14 units a week – an upper limit, not a target.

If your triglycerides and small, dense LDL are creeping up, the driver is often insulin resistance rather than dietary cholesterol, which is why we look at fasting insulin alongside your lipids.

This is how change actually happens. It’s the difference between Dave coming off all medications at 69 and accepting a prescription for life.

 

Supplements with the best evidence

The strongest evidence among supplements is for plant sterols and stanols, which compete with cholesterol for absorption. At around 2 grams a day they can lower LDL by roughly 7.5% to 12%. A normal diet contains far less, so fortified foods are usually needed to reach an effective dose.

Flaxseed may also help – around 30 grams a day has been shown to reduce LDL and total cholesterol. Omega-3 can lower triglycerides, particularly at higher doses. Berberine is promising, but the evidence is still emerging and the mechanism isn’t fully clear.

Which of these is actually worth your money depends on your markers and your medications, which is exactly what proper testing tells us.
